Option #3: Anonymous Submission to IAFF Local 21 Leadership |

If an employee and/or Local 21 member wishes to remain anonymous in reporting a safety concern, they may do so in compliance with OSHA Standard 1960.28(c):
Upon the request of the individual making such a report, no person shall disclose the name of the individual making the report or the names of individual employees referred to in the report.
In the case of imminent danger situations, employees shall make reports by the most expeditious means available.
Local 21 first encourages members to report safety concerns directly to the above listed representatives of the Safety Committee and/or any member of the Local 21 Executive Board. If the safety concern still persists, there is an option to anonymously make a safety concern submission by clicking the button below. The form will be sent to the Chair Officers and the Safety Committee Co-Chair for triage and consideration.
